A. Who Can Enroll
This Capdent NJ plan is available to residents of the entire NJ State. Individuals, couples, families and businesses are eligible. You do not have to have a company or be an employee. The beauty of this insurance plan is that it is accessible to individuals and families. If a family is applying  children are covered to age 19 (25 if full-time students).

 

B. When Will My Insurance Become Effective?
If your application and payment information is received by the 15th of the month, your coverage will be effective on the 1st of the following month. Exceptions can be made. If you are in a rush for coverage and close to the deadline please contact us. We will do our best to place coverage for you right away.


C. Tell Me a Little Bit More About the NJ Capdent plan
In this managed care program, you and your covered family members select a dentist from the CapDent Directory of Participating Providers and receive all treatment from that dentist. Some services are rendered without any cost – others have a minimal co-payment that you pay directly to the dentist. All benefits are available once your plan is effective. What separates this plan from the rest is that there are no forms to fill out, no deductibles, no waiting periods and no annual maximum limit.

D. Annual Deductible & Calendar Year Maximums
There are no annual deductibles or calendar year maximums. This means that the plan will cover all necessary treatments regardless of the dollar amount. You do not have to worry about running out of coverage. With the Capdent plan it is impossible  to run out of coverage.

E. Which Dentists Can I Use?
You can select any dentist in the CapDent Plan Network. Click here to look for dentists. Rest assured that every dentist in the Capdent NJ network has passed a strict 10 point credentials process by HCVO, or HealthPlex Credential Verification Organization. Since they are certified by the National Committee for Quality Assurance, you can be confident that the Capdent NJ plan uses only top notch dentist that have been carefully scrutinized by a non-biased third party.

F. Are Specialists Covered?
Specialists are covered by the Capdent NJ dental insurance plan. You can use any participating specialist in the Capdent NJ network. The specialists are put through the same rigorous credentialing verification system as the general dentists so you can rest assured that you will be under the care of only the most highly trained and professional dental specialists. Should you require the care of a specialist, your co-payment will vary from the amount on the schedule. Care provided by a participating specialist will be discounted  25% less than usual and customary fees. This means that you will only pay 75% of the usual cost of care instead of 100%. Referral forms are not necessary.

G. Are Emergency Dental Services Covered?
Yes. In case of a dental emergency, you will be seen within 24 hours. If  you are away from home with a dental emergency, you will be reimbursed up to $50 for emergency care only.

H. Cancellation Policy
Applicants may cancel at any time, on the condition that dental services were not and will not be used through the plan. If this condition is met, a  prorated refund will be mailed to the insured (minus a $25 service charge and minus the one time processing fee).

J. Can HealthPlex cancel my policy?
No. Healthplex can not cancel your dental insurance policy. However, HealthPlex does reserve the right to amend the plan or rates upon renewal. Of course, should that happen you will be notified in advance (don't worry this has not happened in many years)

L. What if I Have a Complaint?
If you have a complaint, contact a Customer Service Representative to obtain a complaint form and a copy of the complaint  procedure. If your complaint can not be resolved you have the choice of canceling your coverage and recieving a full pro-rated refund.

Q:  Do I need to fill out claims forms?

No. The dentist's office will handle all the paperwork. You will never have the hassle of paperwork .

 

Q: How can I add or remove a family member from my plan?

You can call HealthPlex's service center and they will gladly add or remove family members as per your request.

 

Q: Can I change dentists once I sign up?

Yes. You can change your dentists to any participating dentists as long as you are not in the middle of major work.

 

Q: What if my dentist is not listed in the network?

If your dentist is interested in participating with our program, you can ask Healthplex to send him/her a recruitment package. Applications to participate will be accepted if the dentist meets our credentialing criteria and there is a geographic need for additional providers..

 

Q: How do I know if the dentist I've chosen is a qualified professional?

We guarantee it. All of the dentists in the Capdent network have been credentialed by Healthplex CVO, a Credentials Verification Organization certified by the National Committee on Quality Assurance* in 10 out of 10 credentialing elements. This means that the dentist is guaranteed to have valid medical training and experience, is licensed by NY State to  practice dentistry Etc.

 Healthplex also conducts regular site visits to make sure all offices are well equipped, adequately staffed and are following proper sterilization techniques. If you should have a problem with your dentist, we will rectify the situation or refund your premium! We are the only dental plan specialists that have a triple guarantee attached to our products...

 

Q: If I cancel my policy, will I get a refund?

Yes. You will get a pro-rated refund for the unused portion of the dental insurance plan. An early termination fee may apply. The $39.99 processing fee is non-refundable.

 

Q: Is this dental insurance or a "discount card?"

The Capdent NJ plan is a bona fide dental insurance plan. At NJ Dental Insurance Quotes.Com we do not offer "discount cards" at this time as they are a bad value. There is usually an annual maximum, many services are not covered and there usually is a charge EVEN FOR PREVENTIVE MAINTENANCE! We strongly believe dental "discount cards" are a rip off and we will not take advantage of honest consumers.

We are constantly searching the New Jersey dental insurance market for good plans. At this time it is or firm opinion that compared to our plan, there are no better plans for New Jersey residents.
