How do I enroll in
the Capdent NJ Plan?
First,
review the list of participating dentists and select a
provider that is conveniently located for you and your
family (all family members must use the same dentist).
Then go to "sign up," complete the enrollment form and
enter the name of the dentist you chose. On the same
secure from, you will be prompted for credit card or
check information, whichever is more convenient for you. Please

Q: How does the CapDent plan benefit me and my family?
Capdent programs have no
deductibles to restrain your use of the plan.
Putting off dental care simply means higher future
costs, due to inflation and advancing dental disease.
There
are no annual or lifetime maximums and we offer 100%
coverage on many dental services.
We have
no pre-existing exclusions or special waiting period for
coverage. Teeth extracted prior to this coverage are
eligible for replacement under many plans.
Q: Will I be satisfied with the services of my
participating dentist?
We guarantee it. All of the dentists in
the Capdent network have been credentialed by
Healthplex CVO, a Credentials Verification Organization
certified by the National Committee on Quality
Assurance* in 10 out of 10 credentialing elements.
Healthplex also conducts regular site visits to make
sure all offices are well equipped, adequately staffed
and are following proper sterilization techniques. If
you should have a problem with your dentist, we will
rectify the situation or refund your premium! We are the
only dental plan specialists that have a triple
guarantee attached to our products.
Q:
How do I schedule an appointment?
Simply call your participating dentist after you receive
your ID card and identify yourself as a CapDent plan
enrollee. Visits for routine dental care will be
scheduled within a few weeks of your initial phone call.
If you have a dental emergency, you will be seen
promptly. Should you be away from home with a dental
problem you will be reimbursed $50 for emergency care
only.
Q:
What if I need a
specialist?
Our
programs cover care provided by specialists. Other
programs may have reduced benefits for care given by
specialists.
Q:
What expenses will I have in this plan for general
dentistry and specialty care?
Your costs are clearly noted in the "Patient
Co-payment"
column of the schedule of benefits. Services marked "No
Charge" are rendered with no out-of-pocket expense.
Other services have dollar amounts that you pay directly
to your participating dentist when the treatment is
provided. If you are referred to a participating
specialist
your co-payments will be 25% less than the specialist's
usual fees. Referral forms are not necessary when
visiting CapDent participating specialists. Simply
present your CapDent identification card to verify your
enrollment.
Q: Do I have to
change dentists if my present dentist is not a listed
provider?
Yes.
Only by utilizing participating providers can HealthPlex programs guarantee
the continuing high levels of dental care while
keeping the plan cost low.
Your present provider may already be participating in
HealthPlex's Managed Care Plan.
Q: How will my
treatment differ under this program from my previous
treatment?
You will be treated in the
same professional manner that you are accustomed to
receive from any family dentist. However, with this
plan you will not have to fill out claim forms.
Q: We are a family
who believes in preventive dental care. We visit the
dentist twice a year for cleanings and examinations. Is
this type of maintenance covered by the Capdent NY Plan?
Yes. You and your family are
completely covered for this preventive dental care. We
strongly recommend preventive services and dental health
education. This permits early detection when
treatment can be most effective in preventing dental disease.
Q: Do
I need to fill out claims forms?
No. The dentist's office will handle all
the paperwork. You will never have to obtain forms or
fill them out.
Q: How
can I add or remove a family member from my plan?
You can call HealthPlex's service center
and they will gladly add or remove family members as per
your request.
Q: Can
I change dentists once I sign up?
Yes. You can change your dentists to any
participating dentists as long as you are not in the
middle of major work.
Q: What
if my dentist is not listed in the network?
If your dentist is interested in
participating with a managed care program, you can ask
Healthplex to send him/her a recruitment package.
Applications to participate will be accepted if the
dentist meets the credentialing criteria and there is a
geographic need for additional providers..
Q: How
do I know if the dentist I've chosen is a qualified
professional?
We guarantee it. All of the dentists in
the Capdent network have been credentialed by
Healthplex CVO, a Credentials Verification Organization
certified by the National Committee on Quality
Assurance* in 10 out of 10 credentialing elements.
This means that the dentist is guaranteed to have valid
medical training and experience, is licensed by NY State
to practice dentistry Etc.
Healthplex also conducts regular site visits to make

you should have a problem with your dentist, we will
rectify the situation or refund your premium! We are the

Q: If
I cancel my policy, will I get a refund?
Yes. You will get a pro-rated refund for
the unused portion of the dental insurance plan. If you
have used or will use dental services, you are not
eligible for a refund. An
early termination fee of $25 applies. The $39.99 processing
fee is non-refundable.
Q: Is
this dental insurance or a "discount card?"
The Capdent NY plan is a bona fide dental
insurance plan. At NY Dental Insurance Quotes.Com we
will never offer "discount cards," as they are a bad
value. There is usually an annual maximum, many services
are not covered and there usually is a charge EVEN FOR
PREVENTIVE MAINTENANCE! We strongly believe dental
"discount cards" are not a good deal and we will not take
advantage of honest consumers.
Q: What
will appear on my credit card statement if I pay by
credit card??"
There will be a charge from HealthPlex
for the premium and there will be an additional charge
from "The Oved Agency" (NY Dental Insurance Quotes. com) for
the $39.99 processing fee.
Q: When
will my insurance be effective??"
If you apply
with a valid credit card on or before the 20th of the month, your coverage
will begin the first day of the following month. If you
pay after the 15th, coverage will begin on the first day
of the second month thereafter..
Q:
How
is the processing fee billed? (Together with the annual
payment or separately?)
The processing fee is billed
separately.
There will be a charge from HealthPlex for the premium
and there will be an additional charge from "The Oved

fee.
